It is important that you understand your Health Record/Information.
Usually, every visit to your healthcare provider, a record of your visit is made. Usually this record contains your symptoms, examination, test results, diagnoses, treatment, and a plan for future care or treatment. This information may be called: your health record; your medical record, Physician’s Notes; Physician’s Record or be known by other names used locally. 

The reasons for this record are several. Here are a few:

• It may provide a means of communication among the many health professionals who contribute to your care.
• It will document and describing the care you received.
• It can provide a 3rd party payer a way to verify that services billed were actually provided.

Why is this important to you?  Your understanding what is in your health care record and how the information provided there is used will help you ensure the accuracy of the information kept.  It will also help you better understand who, what, where, when and why others may have need of accessing your health care information.  In this way you can make an informed decision when authorizing discourser to others.  In this regard you do have rights you need to understand.
